Promoting a safe and high quality home-based care model is our highest priority by far. This has guided how we designed every aspect of our program – from our clinical workflows to our technology platform to our org structure and even how we compensate our team members. We carefully measure all aspects of quality and safety on a daily basis and will put our fees at risk to ensure we are enabling a safe and high quality program.

We pride ourselves on the high Net Promoter Score and myriad positive patient testimonials that our program has received to date. We know that your patients and their families are going through a particularly difficult life moment when they are under our care and therefore take every opportunity to make them feel comforted and confident that we are overseeing every aspect of their care journey, at all hours of the day and night.

Recreating facility-level care requires an orchestration of clinical, technology, operational, and supply chain assets. We believe in bringing all of this to our health system customers along with the managed care contracting capabilities that will be required to sustainably offer this important service to your patients.

We know that every market and every care partner looks a little different. Therefore, we focus on offering a flexible operating model. If our care partners have a capability that is key to offering facility-at-home care, we leverage that capability. But we are here for them if they don’t, bringing own capabilities combined with those of our local, regional, and national supply chain partners.

Effort is appreciated, but we believe that performance and results are the only indicators of success. This is why we look to put our fees at risk against a mutually agreed upon set of clinical, safety, and operational metrics. We should only succeed if our customers succeed.
